How to Preserve Text Box or Footer Hyperlinks from Word 2010 to Adobe X?

I'm working in a Microsoft Word 2010 document with the end goal of publishing to Adobe X.  It is critical that all hyperlinks convert to .PDF correctly.  Thus far, I have functioning TOC hyperlinks and functioning internal hyperlinks to bookmarks (think articles to appendix for further reading).

 

However, the final element that is not functioning (i.e. working in .PDF after conversion)  is the text box hyperlinks that return the reader from the appendix back to the article.  I don't care whether it is a text box, a button, text in a footer, etc., so long as the reader has the ability to click (by some means other than just hyperlinked text).  I would use hyperlinked text, but I don't believe I can because I want the textbox (or whatever) to appear at the end of every page, and to not move when text is edited and pagination changes.

 

Thread 515752 is helpful, but doesn't address this final layer.  Thanks in advance!
