PDF form from Excel spreadsheet

I'm wondering if someone can help me with a problem I'm having creating a PDF form.   I'm not sure if what I want to do is possible.

 

I work at a school IT department and we have an excel spreadsheet we use to keep track of classroom tech inventory (computers, printers, projectors, etc.).  We use excel since it is easier for most people to use than creating an Acces database or other database.  While I have a lot of the inventory information in different spreadsheets, every year we ask teachers to submit an inventory.  Lately we've been finding errors in the submitted inventory from teachers (wrong serial numbers, inaccurate information etc.).  This year we would like to distribute a pre-filled form that indicates the inventory we have on record for the teachers to verify, along with add additional things we may not have in our spreadsheets.  I'm consolidating all the inventory information we have into one spreadsheet and want to use that to

 

I'm wondering if there is a way I can pull the information in the existing spreadsheet I'm putting together to create either an individual pdf form for each teacher, or have a generic form which the teachers can then use a drop down item to select their room, and that will populate the information we have from the spreadsheet.  While I think this part shouldn't be to hard to do, I want to be able to distribute the pdf form electronically so they can add any addition items to the inventory and email the completed form back to me.  The trick is to then complie all the info into one excel spreadsheet or a respons document that I can then export out as a spreadsheet.   I don't want to have to manually re-enter the information submitted by the teachers.  I've done something similar with a generic form but not like this where I have specific info to put in different forms for each teacher.

 

I'm just putting this together so I don't have samples of the form or the spreadsheet.  I didn't want to put too much effort into it if I have to do a different way.

 

Any insight or ideas would be greatly appreciated
