Creating a conditional show/ hide script for a checkbox to show/ hide a copied form field

Baffled here - I have a 3 page form in which I have copied multiple fields onto different pages.  On one of the pages I want to create a script for a checkbox where it will show or hide only one of the copied fields.  For example, I have a ship to address on page 1.  The billing address is on page 2 (which I have hidden by default).  I have the check box for 'same as ship to address' set to show the address field if the box is checked.  My problem is when I toggle (uncheck the box) both the billing (copied) and ship to (original) addresses become hidden.  The are the same field only differentiated by the #0 and #1 that adobe adds to the end when there are more than one of the same field.  Should I be running a script in the copied field and not the checkbox?  Hope this makes sense.  Thanks.
